Performer Agreement

By submitting this form and listing my name and email address above I am designating myself the group leader as far as Chamber Pops is concerned, and will receive all communication regarding Chamber Pops. I understand that I am responsible for disseminating that information and ensuring that my fellow group members follow all expectations, and are in the Remind.

Chamber Pops may use my audio/video recording and/or biography for promotional purposes, without revealing any identifying information other than group name, in order to increase awareness of the concert

Every member of my group is required to be at the dress rehearsal ready to play at our call time, and that we are required to be at The Rialto at 5:15 ready to rehearse Sweet Caroline if we choose to leave after our dress rehearsal. I understand that every member of my group is essential and very important and that we cannot effectively rehearse without everyone.

Everything submitted on this form is true and accurate to the best of my knowledge, and I promise to comply with Chamber Pops expectations to ensure everyone has a great concert.

If I did not list a conflict on the dress rehearsal conflicts form that later becomes a conflict Chamber Pops will do their best to accommodate my group, but I understand that this is unlikely and the dress rehearsal schedule, once sent out, will likely not change.  I have put all known conflicts in this field.

I am responsible for disseminating information to my group members and ensuring that they are informed of Chamber Pops’ expectations and that Chamber Pops is informed of any needs that they may have. My group members also understand the terms outlined in this agreement and have committed to abide by them.
